The workers were at the site on behalf of a company that was hired as an independent contractor by Partridge-Raleigh, LLC to perform welding operations and install tanks and piping on dormant wells.
The Chemical Safety Board (“CSB”) did a study and found that the fatality rate within the oil and gas extraction sector was more than eight times higher than the average of all industries in the United States.
After the Partridge-Raleigh explosion investigation, the CSB found that the big lesson coming from the accident is that activity called “hot work”, which is welding, cutting and grinding that could cause ignition on a tank that holds or has held flammable materials, should be handled with extreme caution by workers. Workers killed and injured in the explosion used an unsafe and unreliable technique to check for flammable vapors from crude oil that was once in one of the tanks and for doing the welding itself.
The only person who lived was the foreman at the site, who knew or should have known about more reliable techniques for securing a safer work environment under those circumstances.
